Overview

The Health Outcomes Specialist advances work to ensure health outcomes are not different among the people we serve. This position will partner with agency providers, practice managers and clients to research, plan, implement, evaluate and report on our work to reduce outcome gaps and promote treatment that aligns with clients’ needs across our clinical services. This position will work collaboratively across departments to identify and reduce the differences in outcomes, based on demographic characteristics.

Key Role Responsibilities

  • Works in coordination with the Quality and Community Relations & Engagement Department to develop, implement, and sustain organizational strategies, improve client experience, and ensure consistent, quality of care. Supports leadership in establishing and reporting key performance indicators to monitor progress and maintain accountability.
  • In partnership with Population Health staff, assists clinical teams in identifying priority health measures for the annual performance improvement plan and other organizational initiatives. Use data-driven methods to identify gaps in outcomes across client groups and recommend targeted actions to ensure improved health outcomes are not different among the people we serve. Incorporates structured client input to ensure programs reflect community needs.
  • Reviews data, workflows, protocols, and procedures across demographic categories to identify variations in performance and recommend operational improvements to eliminate differences in client health outcomes.
  • Applies evidence-based practices to guide interventions and program strategies, ensuring that health outcomes are measurable, consistent, and aligned with current research. Stays current with relevant literature and serves as a resource on emerging practices related to improving service delivery. Prepares presentations, briefs, and written materials to support leadership decision-making and organizational planning.
  • Provides support to staff as they test and implement improvement strategies. Supports improvement project leadership through project management and group facilitation, including scheduling meetings, planning agendas, developing meeting materials, facilitating discussions, summarizing decisions and next steps, assisting in communications and deliverable development, and implementation planning.
  • Serves as a subject matter expert on improving quality care across client populations to ensure there are no variations in health outcomes. Delivers staff training on factors that influence health outcomes and how these considerations can be integrated into administrative and clinical operations. Uses strategic communication and health-promotion principles to support agency goals and enhance public awareness.

Knowledge, Experience And Skills

Formal Education and Training

  • RN or LMSW/LCSW-C required
  • MPH or related degree strongly preferred
  • Knowledge of or experience with change management principles and strategies

Experience

  • Three years of direct service delivery within a health care system (ambulatory care setting preferred) or human services organization to an underserved population
  • Five years of racial equity experience, preferably within public health care, human services or community development
  • Three years of population health or related program planning, management and evaluation
  • Experience with visualizing data in concise, meaningful and understandable ways
  • Experiencein preparing issue briefs and presentations and presenting to senior officials
