Overview
Licensed Practical Nurse (LPN) – Clinical Intake Coordinator in a home healthcare - hospice setting, located in Clearlake, California. Start date: 05/04/2026 for a 13-week travel assignment with a weekly pay range of $931 to $981. Day shift, 5x8s (Monday through Friday, 8:00 am–5:00 pm). This role focuses on clinical intake activities, processing referrals, and scheduling within a home health program, with no on-call obligations during the assignment.

Responsibilities
- Serve as the clinical intake coordinator for home health processing and scheduling referrals, ensuring timely entry and routing of referrals to appropriate teams.
- Coordinate incoming referrals and verify insurance coverage to confirm eligibility and streamline admission workflows.
- Manage incoming calls from patients, families, physicians, and care disciplines, delivering accurate information and coordinating next steps.
- Communicate hospitalization status to relevant personnel and follow up on patients admitted to other facilities to ensure continuity of care.
- Coordinate small program components with limited budget/impact, applying clinical knowledge to optimize scheduling, resources, and patient flow.
- Act as a working supervisor for a team or work group as needed, guiding colleagues to meet service goals and maintain patient safety and quality standards.

Requirements
- Active LVN license (Licensed Vocational Nurse) for California; LVN certification required
- At least 1 year of relevant experience in intake coordination, scheduling, or home health processing
- Ability to perform in a day-shift role (M–F, 8:00 am–5:00 pm) within a home health/hospice setting
- Strong communication and coordination skills to interact with patients, families, physicians, and care teams
- Collaborative, organized, and capable of managing multiple tasks in a fast-paced environment
